I hereby authorize any of the schools I have attended to release to Rose Rock Creamery any information contained in their records, including copies of transcripts, faculty comments, etc.
PATENTS
I hereby assign to Rose Rock Creamery (Company) all my rights in and to any inventions which during my employment I may make or conceive, either solely or with others, in the course of such employment or with the use of the employer’s time, material or facilities or relating to the employer’s operations, processes, products or business. I agree to sign, submit, and prosecute all patent applications in the United States and elsewhere, and to execute in favor of Company all further assignments and documents which Company may require, all at Company’s expense. Company shall have no obligations to prosecute any such patent application or to make any payment whatever to me.
DRUG TESTING
I understand Rose Rock Creamery prohibits possession or use of illicit drugs, and I hereby agree to submit to a post accident or reasonable suspicion drug and alcohol test. I understand that agreeing to and passing of the drug and alcohol screening is a condition of employment and failure to cooperate or a positive test will cancel my employment. In signing this form, I consent to the drug and alcohol testing stated above.
BACKGROUND/MEDICAL SCREENING
I understand that any offer of employment by Rose Rock Creamery may be conditioned upon the outcome of a background check of my personal history for which I will be given a separate Disclosure explaining any checks that will be conducted. I further understand that any offer of employment by Rose Rock Creamery may be conditioned upon the satisfactory outcome of a medical examination.
EMPLOYMENT AT WILL
I understand that nothing herein shall be construed as creating an obligation on the Company to employ me for a particular length of time. If hired, my employment with the company is AT‐WILL. Either party may terminate my employment with the Company with or without cause. I further understand that no representative of the company has the authority to make any representations to the contrary, either oral or written.
MISREPRESENTATIONS
It is understood and agreed that any misrepresentation made by me herein will constitute a fraudulent attempt to secure employment and will be sufficient cause for cancellation of this application and/or separation from Rose Rock Creamery or any of its subsidiary companies if I have been employed.
